The details were hard to follow because most of their disagreements were over what each candidate had done to allegedly misrepresent the other's plan.  What is the difference between the health care plans?&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: normal;"&gt;Obama charges that Clinton's plan would 'force' all Americans to obtain health insurance, 'whether they can afford it or not'.  Clinton defends this accusation by stating that her plan would subsidize coverage for those who cannot afford it.  &lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: normal;"&gt;Clinton asserts that Obama's plan would not work, in the same way that Social Security would not have worked if Franklin Roosevelt had made it optional.  &lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: normal;"&gt;Obama defended his position by explaining that people &lt;/span&gt;&lt;span style="font-style: italic; font-weight: normal;"&gt;want&lt;/span&gt;&lt;span style="font-weight: normal;"&gt; health insurance coverage, and if it were affordable, people would invest in it because it's a 'good deal'.  Obama's plan does require that parents obtain health insurance coverage for their children, but also stipulates that young people will be covered under their parents' insurance until the age of 25.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: normal;"&gt;While Hillary Clinton has a great deal of experience in fighting for universal health care, Obama contends that she has not been successful in bringing the powerful players together to make it happen.  Clinton has admitted that she learned a lot after her health care initiative failed in 1993.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: normal;"&gt;The focus of the 16 minute health care discussion that opened the debate was whether or not health coverage should be mandated.  Obama and Clinton have very similar plans aside from this detail.  In either case, the fight for universal health care will be an uphill battle. On July 11th, Spitzer and Paterson directed the NY Insurance Superintendent and the Health Commissioner to begin creating a comprehensive strategy to achieve this goal.&lt;br /&gt;&lt;br /&gt;Spitzer asserted that the plan will incorporate some features utilized by other states but will be designed to meet the needs of New York's uninsured population and health care challenges. In the United States, approximately 2.7 million people are affected by epilepsy, with 200,000 new cases diagnosed each year. By 20 years of age, 1% of the general population will be expected to have epilepsy, and about 10% of patients will have at least one seizure in their lifetime. Treatment for epilepsy involves individual titration of one or more anti-seizure medications to prevent future seizures, minimize or prevent adverse drug effects, and enable seizure patients to lead active lives. &lt;/p&gt; &lt;p&gt;&lt;br /&gt;Pharmacist's Letter is a subscription service that provides recommendations for pharmacists on new developments in drug therapy, and trends in pharmacy practice. Therapeutic Research accepts no advertising and provides objective information for subscribers.&lt;/p&gt;&lt;p&gt;The $20 billion dollar practice of substituting generic antiepileptic drugs for their brand name equivalents will soon be under review by law and policy makers. Therapeutic Research Center, the publisher of Pharmacist's Letter and Prescriber's Letter, informs patients, pharmacists and prescribers on both sides of the argument and provides recommendations on how to provide the best drug care to patients.  A growing controversy over substituting generic antiepileptic drugs (AEDs) for their brand name counterparts will impact pharmacists, prescribers, and patients with epilepsy.  Almost all older anti-epilepsy drugs are already available generically.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;Many newer agents have recently gone generic, and nearly all epilepsy drugs will be available generically over the next few years. Generic substitution in pharmacies is common, legal, well accepted, and can save patients and insurers a lot of money.  Manufacturers of the newer anti-epilepsy drugs stand to lose a lot of money if generic equivalents are dispensed instead of the brand name medicines. Expect both insurance providers and drug manufacturers to try to sway practices to their benefit.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The practice of switching to therapeutically equivalent generics could save more than $20 billion a year. This figure represents revenue that the manufactures don't want to lose and the insurance providers don't want to spend.  Critics of generic substitution for epilepsy drugs point out that epilepsy is different than other conditions. Many anti-epilepsy drugs must be used at very precise doses. Experts call this a "narrow therapeutic window."&lt;br /&gt;&lt;/p&gt;&lt;p&gt;These narrow blood levels are required because slightly too little of the medicine may not control the patient's seizures, and slightly too much of the drug can lead to toxicity.  These critics point to potential variability among the many available generic products and say this increases the likelihood of a problem. For example, Zonegran went generic in 2005 and there are now over 15 different generic zonisamide products.  The FDA does allow a slight variability between generic and brand medications...and among generics...but this usually isn't a problem.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;Despite this, some experts worry that this small variability is too much in epilepsy, where the effectiveness of drugs is heavily dependent on consistency, and argue that generic substitution guidelines for anti-epilepsy drugs should be stricter than with other medications.  There are cases of drug toxicity or breakthrough seizures in patients switching to generic versions of older AEDs like phenytoin (Dilantin), carbamazepine (Tegretol), and others.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;Breakthrough seizures can be devastating to epilepsy patients and may lead to loss of driving or work privileges. But most epilepsy experts say that similar equivalency problems aren't likely with generics of the newer meds because of different drug characteristics.  Most experts agree it is safe and economical to switch to a generic version of a drug when it is therapeutically equivalent. Any variability allowed by the FDA is very small and highly unlikely to impact a patient's therapy.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;Many states refer to the FDA's Orange Book rating for bioequivalence and authorize substituting "A-rated" drugs. Experts say generic substitution can save money for patients and provides equal therapeutic effects when bioequivalent and therapeutically equivalent products are substituted.  "The key for health professionals is to focus on the needs of the patient." says Kristin Weitzel, PharmD, CDE, Assistant Editor for Pharmacist's Letter.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;"Pharmacists are in an ideal position to help epilepsy patients by using their clinical judgment and following their state's substitution laws for epilepsy drugs. For example, there are many AB-rated generics for phenytoin, but it's probably a good idea to keep phenytoin patients on the same product if you can from refill to refill. This can help to decrease any potential variability among epilepsy patients that may have problems."&lt;br /&gt;&lt;/p&gt;&lt;p&gt;"Keep in mind you don't necessarily need to do this with every epilepsy medication, but it may help some patients on older agents like phenytoin and carbamazepine."  Experts at Pharmacist's Letter indicate that many of the anti-seizure drugs are used for many purposes other than treating epilepsy.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;In these instances, the slight variations in the drug products are even less likely to be noticed in a patient. Domenici is an original cosponsor to the bipartisan Craig Thomas Rural Hospital and Provider Equity (R-HoPE) Act of 2007 (S.1605), which was introduced Wednesday by Senators Kent Conrad (D-N.D.), Tom Harkin (D-Iowa), and Pat Roberts (R-Kan.). They, like Domenici, are all Senate Rural Health Caucus members.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The bill will be renamed in honor of the late Senator Craig Thomas (R-Wyo.), a previous sponsor of similar legislation who last week died of cancer. This bill, a Rural Health Caucus effort, takes several steps to shore up health care in rural states like New Mexico and includes a number of provisions supported by Domenici in the past.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;It would extend expiring rural health provisions from the Medicare Modernization Act (MMA) of 2003, the prescription drug benefits law. "New Mexico is among large rural states that are consistently challenged to provide health care to their citizens. The states’ economic condition has improved, but that does not diminish hardships faced by small town hospitals and clinics in attracting and retaining physicians and health care workers,” Domenici said. “In addition, there are many areas where rural hospitals and clinics need special attention because they cannot realistically keep pace with health care demands and rising costs,” he said.&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt; Included in the R-HoPE Act is an extension of provisions—included in the MMA that Domenici first authored and offered as legislation in February 2003—to put rural physicians more on par with urban doctors in terms of Medicare reimbursement levels.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The R-HoPE Act would extend these Physician Fee Schedule Work Geographic Adjustment provisions through Jan. 1, 2010. They are set to expire at the end of this year. Medicare bases physician payments, in part, on geographic adjustment factors.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The purpose of the geographic index is to reflect relative cost differences in a given area compared to the national average. Payments to physicians in rural areas are adjusted downward by this geographic index, thus lowering the reimbursements to rural caregivers.   Domenici also pointed out that the bill contains a provision to benefit community health care centers and rural health clinics, which play an important role in health care delivery for many low-income and rural New Mexicans.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The bill increases the all-inclusive payment rate cap for rural health clinics from $63 to $92 per patient to more appropriately cover service costs. Overall, the R-HoPE Act would extend benefits to all of New Mexico’s 27 rural hospitals that serve a disproportionate share (DSH) of low-income Medicaid recipients and rural hospitals.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;It would, for instance, reinstate the “hold harmless” provisions for small rural hospitals with fewer than 100 beds. It would also provide aid to low-volume rural hospitals, as well as expand Medicare reimbursement eligibility to mental health care providers in rural areas, including marriage and family therapists. The study by Dr. David Chenoweth of Chenoweth &amp; Associates, a North Carolina-based econometrics consulting firm, quantified the cost of three important risk factors – physical inactivity, overweight and obesity – on three major cost components – medical care, workers’ compensation, and lost productivity.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Like Dr. Chenoweth’s studies in seven other states, the cost of these three risk factors in Maine is staggering and warrant immediate attention and corrective action. “Our lifestyle is taking a tremendous toll on our health and economic well-being in Maine,” said Erin Hoeflinger, President, &lt;a href="http://technorati.com/tag/anthem+blue+cross" rel="tag"&gt;&lt;img style="border: 0pt none ; vertical-align: middle; margin-left: 0.4em;" src="http://static.technorati.com/static/img/pub/icon-utag-16x13.png?tag=anthem+blue+cross" alt=" " /&gt;anthem blue cross&lt;/a&gt; and Blue Shield.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;“We knew intuitively that a less active life coupled with weight gain can lead to health problems later in life but this study shows the problem is here and the impact is harming Maine’s competitiveness today.” According to the report, the costs mainly impact employers in the form of more expensive health insurance and lost productivity. Direct and indirect costs of the risk factor physical inactivity totaled $1.289 billion, while risk factors for overweight and obesity totaled $414 million and $863 million, respectively.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Lost productivity (absenteeism, presenteeism, short term disability, and on-the-job injury) in direct and indirect costs totaled $2.09 billion annually based on 2004 data.  The study suggests that without changes in lifestyle, the costs are likely to get worse.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Unless current trends towards increased weight and less physical activity are slowed or reversed, the total impact on Maine’s economy will rise to $3.10 billion in 2009, a cumulative 5-year increase of more than 21%. “We already knew that this epidemic had a big impact on healthcare costs,” says Bill Caron, president of MaineHealth. “This study adds up all the financial impacts, and the bottom line is chilling.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;MaineHealth is working hard on the problem, but we need the help of every person and every organization to address it effectively.” The bulk of the cost of this epidemic is borne directly by employers and taxpayers, but it affects everyone.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;In fact, the report divides the annual cost by the number of Maine residents, giving a per-person per year cost of nearly $2,000. It confirms what past surveys have found; that large numbers of Americans are  seeking out general health information online through Web sites such as Web MD.&lt;br /&gt;&lt;/p&gt;  &lt;p&gt;&lt;br /&gt;It also shows that those online searches are increasingly focusing on the  location and management of personal health information. And while only one in  ten (12%) of Americans currently review their personal medical records on their  health insurance company's Web site, over half say they would like to be able to  check claims and coverage (56%) or access personal records (51%) electronically  in the future.  "It is clear that Americans are ready to enter a digital health care age,"  said George Halvorson, Chairman and CEO of Kaiser Foundation Health Plan, Inc.  American adults overwhelmingly believe (at 72%) that a computer system is  more efficient than a paper system when it comes to managing medical records.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;  &lt;p&gt;But there is still some ambiguity about the relative security of electronic  medical records compared to paper records. The additional benefits and protection from the high cost-sharing under traditional Medicare that MA plans provide is particularly valuable to low-income seniors. A significant challenge facing the Medicare beneficiary population today is the management of care for chronic illness - 82 percent of all Medicare beneficiaries have at least one chronic condition and 65 percent have multiple conditions.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;MA is helping our nation's seniors meet this challenge by providing access to care coordination, disease management programs, and chronic care and wellness programs that are not offered through traditional Medicare.&lt;br /&gt;Proposed funding cuts - which would be in addition to the $13 billion already eliminated from the program in the last Congress - will have devastating consequences.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Reducing MA payments to county-level costs under traditional Medicare would result in millions of MA enrollees losing coverage due to increases in premiums, reductions in benefits, or withdrawal of their MA plan.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Low-income and minority populations rely disproportionately on this program and would be harmed by the loss of the protection MA provides from the high cost-sharing under traditional Medicare. She observed it in a recent clinical psychology class when a student made the frank disclosure that she suffered from bipolar disorder. "Other students, who had previously been engaged and excited about learning about mental illness in the abstract, became extremely uncomfortable, refused to make eye contact with her, and were quiet and withdrawn," Stier says.&lt;br /&gt;&lt;br /&gt;That enduring fear of mental illness is precisely what UC Berkeley psychologists - under the tutelage of Stephen Hinshaw, chair of the Department of Psychology - are working to combat.&lt;br /&gt;&lt;br /&gt;Indeed, Hinshaw's new book, "The Mark of Shame: Stigma of Mental Illness and an Agenda for Change" (Oxford University Press, 2007) could well boost compassion for the mentally ill from the grass roots to the U.S. Capitol. As Congress considers legislation that would require the same &lt;a href="http://www.fast-health-insurance.com/"&gt;health coverage &lt;/a&gt;for mental illness as for physical ailments, lawmakers are becoming more receptive to evidence of the injustices suffered by those with disorders ranging from depression and attention-deficit/hyperactivity disorder to schizophrenia and autism.&lt;br /&gt;&lt;br /&gt;According to Hinshaw's book, those who suffer from mental illnesses continue to face housing and job discrimination, as well as hurdles when voting, obtaining a driver's license and maintaining child custody. "As a result, they lose out on major life opportunities," he says. Indeed, the public perception of serious mental illness is more negative today than it was a half century ago, despite significant advances in education, medication and psychological therapies, says Hinshaw, who will give a public reading from his book at 7 p.m. on Monday, Jan. 29, at Cody's Books at 1730 Fourth St. in Berkeley. In his book, Hinshaw uses scientific, cultural and qualitative evidence to explain the historical roots of the stigma of mental illness, how it plays out on a daily basis, and what can be done to remedy this continuing problem.&lt;br /&gt;&lt;br /&gt;He warns that the stigma of mental illness will persist until a "civil rights movement" establishes equality for the mentally ill. As part of their research into the stigma of mental illness, Hinshaw's team of graduate psychology students are looking into how the mentally ill are depicted in the news media, public versus hidden attitudes about mental illness, and the responses of grade schoolers and their parents to film clips about mental illness.&lt;br /&gt;&lt;br /&gt;They are also studying how interpersonal dynamics change when one member of a group is led to believe that another has been referred for mental health care and may have a diagnosis of schizophrenia or depression. Meanwhile, studies show that the stigma of mental illness prevents tens of millions of Americans from seeking treatment.&lt;br /&gt;&lt;br /&gt;A 2005 government-funded study, the National Comorbidity Survey Replication, found that fewer than half of those who reported mental illnesses during that year sought treatment, and those who did waited at least a decade, by which time they were more likely to have developed additional problems. Comorbidity is the simultaneous occurrence of two or more illnesses. "Given the negative perceptions of mental illness, and the shame, it is not surprising that people with mental disorders delay seeking help for decades," Hinshaw says.&lt;br /&gt;&lt;br /&gt;"Concealment remains a major means of coping." Largely to blame, Hinshaw says, are primal human fears of members of an "outgroup" threatening stability and survival, alarmist portrayals of the mentally ill in the media, and the failure of politicians and the &lt;a href="http://technorati.com/tag/health+care" rel="tag"&gt;&lt;img style="BORDER-RIGHT: 0px; BORDER-TOP: 0px; MARGIN-LEFT: 0.4em; VERTICAL-ALIGN: middle; BORDER-LEFT: 0px; BORDER-BOTTOM: 0px" alt=" " src="http://static.technorati.com/static/img/pub/icon-utag-16x13.png?tag=health+care" /&gt;health care&lt;/a&gt; industry to give equal footing to mental health insurance coverage.&lt;br /&gt;&lt;br /&gt;While reports in newspapers and on television often depict the mentally ill as dangerous and unpredictable, the reality is that only a sub-group of people with mental illness are likely to be violent, Hinshaw argues. Indeed, people with mental illnesses are more likely victims than victimizers. "Neglect and exclusion impede adequate research funding and clinical care, fueling the myth that mental illness is lifelong, hopeless and deserving of revulsion," Hinshaw writes in his book. Hinshaw, who previously has written about his father's lifelong struggle with misdiagnosed bipolar disease and is a longstanding investigator of developmental psychopathology, is particularly committed to combating the sense of shame that prevents the mentally ill from seeking help. "While there is a strong recognition that prejudice based on race, ethnicity and gender is a social problem that we need to understand and address, such recognition has not been given to prejudice - or its consequences - based on mental illness," says UC Berkeley psychology professor Rodolfo Mendoza-Denton, whose research also deals with stigma, but as it pertains to race and ethnicity.&lt;br /&gt;&lt;br /&gt;"This is why professor Hinshaw's book is so important." "The Mark of Shame" demonstrates how mental illness has historically been marked with ignorance, superstition and even torture, from the early practice of boring holes in heads to release evil spirits to inhumane post-Renaissance asylums, to eugenics and Hitler's genocide programs. Moreover, the deinstitutionalization strategies of the latter part of the 20th century, Hinshaw says, emptied most mental hospitals.&lt;br /&gt;&lt;br /&gt;As a result, huge numbers of the mentally ill today live on the streets or are warehoused in jails and prisons.&lt;br /&gt;&lt;br /&gt;Meanwhile, conditions in psychiatric wards at the few remaining public hospitals are plagued by understaffing, assaults, suicides, drug trafficking and the excessive disbursement of psychotropic medicines, he says. Daines, M.D. today called for enactment of a plan to cut New York's uninsured population in half over the next four years.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;The plan, included as part of the proposed state budget, would expand access to &lt;a href="http://www.fast-health-insurance.com/"&gt;health insurance &lt;/a&gt;to 400,000 uninsured children by increasing the State's Child Health Plus program and simplifying Medicaid rules to reach both uninsured children and the 900,000 uninsured adults who are currently eligible for coverage. "By expanding the Child Health Plus Program and removing bureaucratic hurdles that keep eligible New Yorkers from getting on and staying on Medicaid, we will be able to cut New York's 2.6 million uninsured population in half over the next four years," said Dr. Daines.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Daines testified about increasing access to health care insurance Friday at a hearing held by the Assembly Health, Insurance and Labor committees.&lt;br /&gt;In his testimony, Dr. Daines noted that the proposed state budget would expand Child Health Plus to cover kids in families up to 400 percent of the federal poverty level, with co-premiums based on a sliding scale.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;"Our plan ensures that every family in New York will be able to provide their children with the &lt;a href="http://technorati.com/tag/health+insurance" rel="tag"&gt;&lt;img style="BORDER-RIGHT: 0px; BORDER-TOP: 0px; MARGIN-LEFT: 0.4em; VERTICAL-ALIGN: middle; BORDER-LEFT: 0px; BORDER-BOTTOM: 0px" alt=" " src="http://static.technorati.com/static/img/pub/icon-utag-16x13.png?tag=health+insurance" /&gt;health insurance&lt;/a&gt; they need," he said.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;"There is a direct correlation between the ease of application and enrollment procedures and participation in health insurance programs," Dr. Daines testified before a joint Assembly committee hearing. "That is why the Department is undertaking a top-to-bottom review of the enrollment and renewal requirements for these insurance programs and seeks to implement measures that make it easier for eligible families to obtain and maintain coverage."&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Increased access to Medicaid will allow more individuals to receive primary and preventative care and reduce reliance on expensive hospital care, a goal that will also be reached for children with increased access to Child Health Plus, which would provide health insurance to virtually every child in the State. Both actions will ultimately result in saving the State hundreds of millions of dollars from reduced charity care in emergency rooms, said Dr. Daines.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;"Ultimately," he said, "the State will develop a plan for affordable, universal health insurance for all New Yorkers. However, unless we first restructure our health care delivery system to lower health care costs we will force an undue burden on families, businesses and government to cover the cost of universal coverage." As one way of increasing the accessibility to public health insurance programs, Dr. Daines said the Department is exploring ways to streamline and simplify guidelines and also apply uniform benchmarks to the Medicaid, Family Health Plus and Child Health Plus programs. With a broad coalition representing  labor, business, consumer, insurer, nurse and physician, ethnic, senior and  hospital groups, Governor Schwarzenegger today discussed how focusing on personal  responsibility will make California a leader in prevention, lower health care  costs and save lives. "It's great to be joined here today with people from groups that don't always  see eye to eye on health care issues, but they all support our emphasis on  prevention because it is the least expensive and most effective way to lower  &lt;a href="http://technorati.com/tag/health+care+costs" rel="tag"&gt;health care costs&lt;/a&gt;" said Governor Schwarzenegger. Under the Governor's reforms, health  plans and insurers will be required to offer benefits and incentives, including  premium reduction, if certain goals are met. Healthy Actions programs are not  punitive. Individuals who are overweight, smoke or suffer from diabetes, for  example, will not be charged higher premiums. Healthy Actions Programs will be  offered through Medi-Cal, CalPERS, Healthy Families and commercial coverage.&lt;/p&gt; &lt;p&gt;&lt;strong style="font-weight: normal;"&gt;&lt;u&gt;&lt;/u&gt;&lt;/strong&gt;Increase patient safety: Under the Governor's  reforms, California's health care providers will work with the state to reduce  errors and make patients safer. His proposal includes:&lt;/p&gt;o        Requiring  e-prescribing by all providers by 2010.&lt;br /&gt;o        Issuing a call to action  for California's health care facilities to implement critical safety               measures  related to infection control, surgical errors and adverse drug events by 2008.&lt;br /&gt;o        Enacting new health care safety measures and reporting  requirements.&lt;br /&gt;o        Establishing an Office of Patient Safety within the  Department of Public Health.  &lt;p&gt;A 10 percent reduction in medical errors will reduce health care costs by an  estimated $450 million per year. In 2005 the Institute of Medicine estimated the  total national costs of medical errors to be between $37.6 billion and $50  billion annually. The Governor proposes that all prescriptions be electronic by  2010. Currently, medical errors cost California $4.5 billion a year and result  in thousands of preventable deaths.&lt;/p&gt; &lt;p&gt;&lt;strong style="font-weight: normal;"&gt;&lt;u&gt;&lt;/u&gt;&lt;/strong&gt;Fight obesity through public awareness: The  Governor's reforms take on the obesity epidemic using the same approach as  California's internationally recognized anti-tobacco campaign. Strategies  include:&lt;/p&gt; &lt;ul&gt;&lt;li&gt;A public education campaign to encourage physical activity and motivate  Californians to make healthy lifestyle choices.  &lt;/li&gt;&lt;li&gt;Local assistance grants and state technical support to promote "community  makeovers" that make healthy choices the easy choice.  &lt;/li&gt;&lt;li&gt;Helping employers integrate wellness programs into employee benefits plans  and worksites.  &lt;/li&gt;&lt;li&gt;New standards and policies to improve nutrition in our schools, and  school-based wellness activities established by local school districts and  communities. &lt;/li&gt;&lt;/ul&gt; &lt;p&gt; &lt;/p&gt; &lt;p&gt;&lt;strong style="font-weight: normal;"&gt;&lt;u&gt;&lt;/u&gt;&lt;/strong&gt;Promote better diagnosis and self-management to keep diabetes in check:  The Governor proposes a five-year Diabetes Prevention and  Management Initiative to: Screen Medi-Cal beneficiaries for diabetes; promote  self-management of diabetes; reduce the impact of diabetes on the health of  Californians; and reduce the diabetes-related costs on California's health care  system.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;&lt;u&gt;&lt;br /&gt;&lt;span style="font-weight: bold;"&gt;&lt;/span&gt;&lt;/u&gt;&lt;/strong&gt;&lt;span style="font-weight: bold;"&gt;&lt;/span&gt;Help Californians Quit Tobacco:  The Governor's reforms will help an estimated 46,000 people get access to  programs that can help them stop smoking, and double the number of Californians  using their health insurance cessation benefits.&lt;br /&gt;&lt;/p&gt; &lt;p&gt;The benefits of achieving a healthy weight and staying fit include: lower  health care costs, more lives saved and improved student achievement. A 2005  study by the California Department of Health Services estimates that if the  number of physically active Californians increased by 5 percent to 52.5 percent,  we could realize a reduction in direct and indirect medical costs of $1.3  billion/year or $13 billion over ten years.&lt;br /&gt;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;&lt;/p&gt;&lt;p&gt;The Governor's reforms can help  achieve a 10 percent reduction in heart disease, the number one killer of  Californians, over 15 years. This could translate to 7,000 lives saved annually  if healthy lifestyles were broadly adopted in California. The physical  well-being of students has a direct impact on their ability to achieve  academically. A new study has identified simple steps that hospitals can take to speed life-saving treatment. The research, which was led by Elizabeth Bradley and a team of researchers that included David Magid, MD, MPH, &lt;a href="http://technorati.com/tag/kaiser+permanente" rel="tag"&gt;Kaiser Permanente&lt;/a&gt; emergency medicine physician and researcher, was announced today by the New England Journal of Medicine at the American Heart Association’s annual conference.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;It outlines simple hospital strategies that work when patients arrive at the emergency department needing percutaneous coronary intervention or balloon therapy. The team is challenging hospitals to improve what has been dubbed “door-to-balloon” times.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Those strategies include:&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Having emergency medicine physicians activate the catheterization lab (cath lab), the hospital room where the balloon treatment is performed.&lt;br /&gt;&lt;br /&gt;Activating the cath lab with one-call central page operators who can mobilize the entire medical team at once.&lt;br /&gt;&lt;br /&gt;Activating based on pre-hospital electrocardiograms while the patient is still en route to the hospital as opposed to waiting until the patient arrives.&lt;br /&gt;&lt;br /&gt;Having interventional cardiologists on location within 20 – 30 minutes.&lt;br /&gt;&lt;br /&gt;Using real-time data feedback with emergency department and cath lab staff.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;The study of 365 hospitals found that only a handful of hospitals use some of the strategies and none uses all. Researchers say implementing this simple set of effective and feasible strategies holds promise for substantially improving patient care.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Balloon therapy has become the preferred approach to treating certain kinds of heart attacks. While, hospitals have been seeking ways to improve their door-to balloon times, overall performance at most hospitals lags behind the national standard of 90 minutes or less. Dr. Magid practices at Exempla Saint Joseph Hospital and as part of this study partnered with the hospital to adopt the life-saving strategies.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Saint Joseph Hospital is one of the first hospitals in the country to make these important improvements. “The great point about these strategies is that most involve little or no cost to implement,” said Magid.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;“The study offers a terrific guide for hospitals to follow as they push to improve door-to-balloon times and the quality of care for patients.”&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;This study is part of the Health Care Quality Improvement Project initiated by the Centers for Medicare and Medicaid Services, which has encouraged identification of quality improvement projects to improve patient safety.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;It also is one of more than 100 research projects conducted every year by the Kaiser Permanente Colorado researchers.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Kaiser Permanente is the largest private health care provider in Colorado. According to a new report issued today by the two consumer organizations, call center representatives for companies offering Part D coverage remain unable to answer basic consumer questions regarding how the benefit works.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;The report found that the call centers frequently could not provide the information people with Medicare needed to make appropriate enrollment decisions. In “Getting the Runaround: Problems with Obtaining Accurate Information from Part D Plans,” the two advocacy groups state that plan call centers are expected to serve as a crucial link to information about Part D coverage, but “independent surveys and anecdotal reports from counselors show serious lapses in the ability of Part D plans to provide accurate and timely information and to successfully guide people with Medicare seeking to access the benefit.”&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;“Good advice is the only antidote to the convoluted prescription drug benefit that older and disabled Americans are forced to navigate,” said Robert M.
